What happens when the uninsured go to the emergency room?

The U.S. Census Bureau reports that an increasing number of people are now unable to afford medical insurance. Some 47 million people do not have medical insurance. When they begin to fall sick, there is nothing that can be done if money is short. When it comes to a choice between food on the table and treatment, most people decide to eat. They hope they will get better. When health does not improve, there is no improvement in the choice to be made. If treatment remains unaffordable, they have to wait until their sickness worsens to the point it can be considered an emergency. At this point, people decide to go to the emergency room at their local hospital. Federal law is very clear. Hospitals are under a positive legal obligation to treat everyone who walks in through the door. It does not matter whether the emergency is real, in the sense of a traffic accident inflicting unexpected injury, or to some extent manufactured, where the condition only becomes an emergency because of a deliberate delay. People must be given treatment. The difficulty is that most of the uninsured cannot afford to pay their bills. The hospitals can and do issue invoices for the treatment given and drugs supplied. This is also a part of the law. People have a responsibility to pay for their treatment. But hospitals are realistic about their chances of collecting. Continued pursuit for payment usually results in bankruptcy and the creditors only get a few cents in the dollar. So, hospitals make a rational decision. They spread all the unpaid bills among all those who can pay. In other words, whether you are paying out of your own pocket or you are relying on your own health insurance to pay for your treatment, a percentage of every hospital's bill is a provision against bad debts from the uninsured. The irony is that everyone who is insured is also insuring all the uninsured for their emergency room visits. If you have been wondering why your own health insurance premiums have been going up so sharply of late, it's because there is a wave of uninsured people going to the emergency rooms around the country. The health insurers are having to pay more and this additional cost gets passed on in the premiums. Is it going to get any better? No. It's actually going to get worse. Yet the regulators responsible for keeping Australia safe for the Australians has directed that one advert be removed from public display. The controversy over the decision has been impressive. Advocates of free speech have clashed with the more conservative elements in society that prefer not to have their eyes assaulted as they drive down the highways.
And the reason for all this excitement? As a part of a perfectly legitimate campaign to persuade more men to seek treatment for erectile dysfunction (or even quietly to go on to the internet and buy viagra online) adverts have been asking the question, "Want longer lasting sex?" Spam remains profitable, but only just

One of the more fascinating things you can think about when you're bored is why the spam is so different depending on where you have accounts. Perhaps I'm just lucky, but I get very little spam through my ISP. Mostly, it's just to persuade me to buy viagra and other more obviously fake ways of producing sexual enhancement. I suppose the way I trawl the web to find stories to write about here sells my name as someone desperate to find a way of overcoming sexual inadequacy. But, when it comes to Gmail, my inbox is more evenly divided between viagra and gambling sites. And then come the yahoo accounts (I have several for different purposes). Almost without exception, I am flooded by the Nigerian scam mail. It seems the spammers target different user groups depending on the mail servers they use. A research team based at the University of California has been digging into the problem - it's completely fascinating to see how some research teams spend their time. Anyway, this team decided to try estimating how much money the spammers made out of persuading people to buy Viagra. Their guess? $3.5 million a year. How did they come up with this number? Well, like cunning hackers, they wormed their way into the Storm botnet. For the uninitiated among you, this is one of the control centers for all those hacked computers around the world. Storm lets you send out millions of e-mails. To monitor responses, they set up two websites of their own to promote. One offered to sell viagra. The other was designed to mimic infecting the users with trojans - the same little bits of code that allow spammers to hijack machines in the first place. Both were actually harmless but counted the traffic and downloaded benign bits of code. Now comes the exciting bit. They sent out almost 470 million e-mails. There were 350 million to promote the viagra site with 10,500 people responding and 28 people attempting to buy Viagra in quantities worth more than US$100. So the low conversion rate did not mean low profits. By scaling up this hit rate, the research team arrived at their annual estimate for gross revenue. But it's actually quite expensive to send out all this spam so the only way the operation pays is if the spammers also run the sites they promote. The infection site was more efficient, converting an average of 6,000 PCs a day to clones. OK, so now you know who to blame for some of that spam you have been receiving, you can all get your own back by e-mailing the research team which is based at the campuses at Berkeley and San Diego.

There was something important I had to tell you

I know it was important because I tied this knot. I always tie a knot when I need to remind myself to remember something. However, the most recent research suggests an alternative - that there is a loss of an enzyme called neuronal nitric oxide synthase (nNOS). This prompted the researchers to turn to an unlikely medication for treatment - viagra. As you know, viagra works by allowing the smooth muscles in arteries to dilate. The research team has bred lab mice with muscular dystrophy (scientists are really clever in a macabre kind of way). They examined the blood vessels before and, after light exercise, found the vessels had constricted, reducing the flow of oxygenated blood. But when these mice were given viagra, they were able to exercise without tiring. Preliminary testing in human volunteers shows that they too lack nNOS. The hope is that treatment with viagra as an already approved drug will allow people with muscular dystrophy to move around with getting so tired. However, the fact that this treatment works in mice is no guarantee that it will work in humans. The further difficulty is that if humans are able to exercise more, this may accelerate the breakdown in the muscle cell structure. There needs to be considerable testing before this treatment can be approved. But it does at least offer the chance that the lives of these people can be improved while we wait for gene splicing therapy to come along in the future. What to do about jet lag?
